Introduction
The Casualty Assistance Kit was created by Stephen Cobb, Adjutant and Finance Officer of the Greater Washington Chapter 353, with inspiration and assistance from George Merlin Gallagher, former member and officer of the Chapter, in September 2003. Desktop publishing and Internet-related support was provided by Tanya Cobb, Webmaster and President of our Ladies' Auxiliary Unit. This Casualty Assistance Kit is intended to be completed by veterans and/or retirees and their spouses. Please complete and keep it in your files for future use by your survivors.
The Casualty Assistance Kit is not a copyrighted document. It may be freely used, duplicated, and distributed in print and electronic form by anyone with the following two conditions. First, the Casualty Assistance Kit must not be provided to anyone for a fee. Second, the information provided on pages 1 and 2, and the footnote information on each page referring to the material courtesy of Army Echoes magazine must not be removed or modified in any way.
For your convenience, the Casualty Assistance Kit is available in several formats. Please note that the files available for download did not contain computer viruses or other malware at the time they were uploaded to the Internet.
